Remote SQL Connection to Custom Fields stored data

Hello All,
What I am trying to do is open a remote MySQL connection from a website to my account center database. Once the database connection is opened, I would like to find the correct user's information that is entered in the custom tables when a user is required to fill them out.

What I need help finding out is where is the location of the table that the custom field input data is stored in and what is the tables name?
