Adding ID Protection

Hey All,

i am trying to convert my domain order page to like the following one,

I have copied all the code in, and for some reason my ID Protection appears at the very top of the page, See image below,



The code entered is a direct copy & paste from the instruction page.

If it is something simple i am sorry for that, I am not a coder by any means :)
