Cant seem to get past domain registration screen.

Hello,

We have tried everything under the sun and the moon and we cant seem to get past the domain registration screen using some of the order templates. Some work and some dont. Cant seem to figure it out. You all helped us with this a few years ago I recall but I cant remember how to do it. HELP PLEASE.....
